For the safety of all pets in our care, I understand that my pets must be up to date on necessary vaccinations, free of intestinal parasites, fleas and ticks to enter and stay in this facility.
I understand my pet will be administered a CAPSTAR flea pill on the day of entry to the facility whether my pet is on a flea control product or not. This is to prevent any possibility of flea introduction to our facility. Pets who are found to have ticks or evidence of mites will be treated immediately. I understand that these parasite treatments will be at my expense.
I understand that pets boarded away from home are under some stress from being in a different environment, different routines, and absence of family members. We minimize this stress by maximizing personal attention, providing soothing music, natural pheromone plugins, and a nutritious, high-quality, highly digestible diet. I understand that even with the best care, it is possible that my pet(s) could become ill, injured or experience an emergency while boarding at this facility. In the event that any of this happens, I understand that this boarding facility will call on the veterinary medical staff of Wilkinson Animal Hospital for my pet(s) medical needs. If treatment, medicine, or doctor / nurse care is needed, I give my permission to do whatever is deemed necessary for my pet. I understand that in the event that extensive care is needed, the staff of the hospital will make every attempt to contact me at my emergency number as soon as any emergency situation is stabilized. I understand that this medical care will be at my expense.
I agree to board my pet(s) at the boarding facility within Wilkinson Animal Hospital and agree to the written policy, which I have read and understood.
